Sapphire Wellness is a membership-based practice which means we do not take insurance. This allows us to work outside the regular model of medicine, which is to see as many patients as possible in a day, and usually for just 15 minutes. But not here! Your membership will allow you to spend an hour with Lisa, initially and 30 minutes or more at each additional appointment. You have direct access to the provider and will have check-ins, that are included in your membership fee. Basically, we don’t want to see you once and then send you on your way with a new medication or plan. We know that you may have questions or concerns that develop along the way and you shouldn’t have to wait until your next appointment for the answer to a simple question!

The advantage of the membership-based model is that our office is small and the goal is to get to know YOU! We want to know your family history, your personal history, all your medications, all your stressors, because this allows us to get to the crux of the issue and can help you get to your goals faster by seeing us more frequently.

Our commitment to you is our time and expertise. The traditional medicine model wants to determine how sick you are and how much you will cost your insurance company. Direct-care medicine allows you to make an investment in yourself and know that your provider has the time to make your investment pay out in dividends!

Another great service we will provide is a super bill that allows you to file the claim with your insurance. You may be able to use FSA funds to cover the membership expenses as well. NOTE: this does vary by plan so you must check with your plan prior to using those funds.

Membership Registration

Initial Consultation

Get ready to take control of your health and have a personalized, evidence-based plan of care consult. This is actually a 2 appointment process. The first appointment will be your full intake with a meet and greet with Lisa, a thorough patient history, family history, list of concerns and recommended optimization lab panels. The second appointment will be reviewing the labs and discussion of the optimization plans that Sapphire Wellness has available. This data-driven assessment will allow for a personalized blue-print that gives you a guideline to attain the results that you have been desiring. Following this consultation, you may elect to enroll in one of our specific care plans to support and sustain the transformation and health-aligned goals. Note: Labs are included in this consult and as part of the initial appointment cost of $450.

Pearl Plan

Our basic plan gives the patient foundational support that will include lifestyle coaching, and access to group classes. This plan will not focus on prescriptions, but will provide a framework for lifestyle changes that produce a blueprint for health goals. A three month commitment is required for this plan.

Sapphire Plan

This comprehensive plan gives you the foundational support that includes intervention that is whole body and includes prescription support. You will have access to our member hub that will help you find recommendations on supplements and vitamins, dietary plans, weight training, and other resources that can help you become your best you! Our member hub will also include provider check-ins on a weekly basis, on-going lab monitoring, and compliance tools. You will also have first access to group classes that can bring women together to help build a community of support as we all work to create a healthier life for ourself and our community. A three month commitment is required for this plan.

Diamond Plan

The Diamond Plan provides the patient with the 1:1 clinical experience that is is nothing short of having your own personal provider on-call, optimizing all of your health goals. This concierge plan will give the patient access to everything in the Sapphire plan, while also working with other health partners that give the patients the most well-rounded, comprehensive treatment plan in the Southeast!

Health Consults

These appointments are ideal for those seeking more information on a recent diagnosis or a second opinion on labs, radiology reports, referrals, etc., from an outside provider. The appointment typically lasts 30 minutes and is meant to give you education on your current needs. There will be no medications prescribed at this visit and will be a charge that is separate from the Pearl, Sapphire, and Diamond Plans. Cost for this second opinion visit is $150.

Forms Fee

It seems everything requires proof these days, and forms abound! If you need further documentation than the super bill provides, or we have to manage prior authorizations to get your commercial insurance prior authorizations processed for the weight loss meds Saxenda*, Wegovy* and Zepbound*, you will be charged an additional $50 fee.

*Note: all major insurance plans require a prior authorization for Wegovy and Zepbound and Saxenda at this time.
